Many of our clients have implemented Supplier Diversity Programs to assist them in creating reports and maintaining records. These records are required to verify the business size and classification of their suppliers and potential suppliers in accordance with government regulations and prime contractor requirements.

To further our commitment as a prime contractor, we have implemented a Supplier Diversity Program at Excellence In Motivation, and are asking all current and future suppliers, subcontractors and resellers to assist us by completing the attached Business Information & Classification Form. Also attached is the worksheet EIM uses to gather second tier spend information from our vendors.

Please take the time to read the definitions on the second page of the form to determine if you are classified as minority-owned business. We will also need proof of certification of your minority status. If you know you qualify but are not certified at this time, we have also listed organizations you may contact to receive your certification.
